Registration Information for 2010-2011 School Year

Registration Fees, Student Fees and all required documents should be mailed to or dropped off at the Frassati Catholic Academy school office.

Students from Santa Maria del Popolo, Saint Mary of the Annunciation and Transfiguration parishes have registration priority. All registration is on a first come, first served basis. Registration Fees and Student Fees will be refunded if enrollment is not accepted.

Highlight of documents required for 2010-2011:

• Student Registration Form (must be completed by all families and includes one family information page and one student information page for each student registering)
• Transportation Registration Request Form
• State required Medical and Dental form for all students entering Grade 6 (must be on file with the school office prior to the first day of school)
• Records Transfer Release (allows all birth certificate, baptismal certificate, medical, eye and dental records, transcripts and test scores to be transferred – this applies only to 2009-2010 students of Santa Maria del Popolo School, Saint Mary of the Annunciation School or Transfiguration School).
• New Students (students not transferring from a Participating Parish School) must provide a copy of birth certificate, baptismal certificate and a records transfer release for previous school as well as completed Medical, Eye and Dental forms. An enrollment meeting with the Principal will be scheduled.
